Description
- 1. Technisches Gebiet
- Die Offenbarung betrifft Verbindungselemente für Segmente modularer chirurgischer Implantate als auch modulare chirurgische Implantate, und insbesondere solche modularen Implantate für den Ersatz, die Reparatur und/oder Verstärkung von hartem bzw. festem Gewebe.
- 2. Beschreibung des Standes der Technik
- Chirurgische Implantate werden häufig dazu verwendet, hartes Gewebe im Körper eines Patienten zu ersetzen, zu reparieren oder zu verstärken bzw. zu vermehren, und sie werden dann eingesetzt, wenn ein Körperteil auf Grund von Trauma oder Krankheit verstümmelt oder beschädigt ist. Chirurgische Implantate werden ebenso als Ersatz oder Verstärkungshilfsmittel bei der kosmetischen oder plastischen Chirurgie verwendet.
- Verschiedene Materialien sind aus dem Stand der Technik bekannt, die zum Herstellen chirurgischer Implantate verwendet werden können. Beispiele solcher Materialien umfassen Chromkobaltmolybdenlegierungen, rostfreier Stahl, Titanlegierungen, Keramiken und verschiedene Polymere und Polymer/Keramik-Verbundstoffe. Die für chirurgische Implantate verwendeten Materialien sollten biokompatibel sein, d. h. sie dürfen nicht auf nachteilige Weise lebendes Gewebe beeinflussen, und das von dem lebenden Gewebe erzeugte Umfeld darf nicht auf nachteilige Weise die Prothese beeinflussen.
- In vielen Fällen müssen die chirurgischen Implantate Körperteile, wie z. B. Knochen, verkleiden, ersetzen oder verbessern bzw. verstärken, wobei die chirurgischen Implantate fest sind. Folglich bestehen die Implantate normalerweise zumindest teilweise aus festem Material. Zusätzlich sollte die Größe und Form des Implantates der Größe und Form des zu ersetzenden oder zu verstärkenden Körperteils nahe kommen. Da jeder Patient einzigartig ist, besteht ein häufiges Problem für Mediziner darin, ein Implantat zu finden, das die vorbedingte Form und genaue Passung besitzt, speziell da, wo feste Implantate erforderlich sind.
- In einigen Beispielen wird ein speziell an den Patienten angepasstes chirurgisches Implantat dadurch gebildet, indem eine Form des Körperteils des einzelnen Patienten hergestellt wird, und anschließend ein speziell an diesen Patienten angepasstes chirurgisches Implantat aus der Form gewonnen wird. Ein damit mit diesem Verfahren zusammenhängendes Problem ist es, dass es sehr zeitaufwändig ist aufgrund der Arbeit, die zum Herstellen der Form und dem anschließenden Gussformen des Implantates erforderlich ist. Auf diese Weise ergeben sich ebenso höhere Kosten für den Patienten. Bei anderen Verfahren wird ein chirurgisches Implantat nach der ungefähren Größe und Form des Körperteils gebildet, und anschließend ausgehöhlt oder skulpiert, um so die Bedürfnisse des einzelnen Patienten zu befriedigen. Das in Zusammenhang mit dem Erzielen der genauen Form und Größe stehende Problem wird dadurch verstärkt, wenn das chirurgische Implantat aus festem Material besteht, d. h., ist das Implantat bereits gebildet, so ist es zeitaufwändig und schwierig, Anpassungen bzgl. seiner Größe und Form vorzunehmen. Jegliche Fehler, die beim Anpassen der Konfiguration unterlaufen, sind schwierig zu korrigieren, und können die Herstellung eines vollständig neuem Implantates erforderlich machen.
- Standardisierte, bereits gebildete feste Implantate sind ebenso eingesetzt worden. Diese standardisierten Implantate berücksichtigen gewöhnlicherweise jedoch nicht Unterschiede hinsichtlich des Knochen- oder Gewebeaufbaus einiger Patienten, und müssen ebenso einzeln in die erforderliche Form ausgehöhlt werden, um eine ordnungsgemäße Passung sicherzustellen.
- Zusätzlich werden in einigen Situationen diese Implantate zur Implantationsstelle entlang einem kurvenreichen oder gekrümmten Weg eingeführt. Das Einführen eines festen Implantates entlang einem solchen Weg kann umgebendes Gewebe beschädigen, oder kann selbst eine Fraktur des Implantates verursachen.
- Deshalb besteht eine Notwendigkeit für chirurgische Implantate, die aus einem festen Material gebildet sind, und die auf Bedürfnisse der einzelnen Patienten Rücksicht nehmen, ohne es erforderlich zu machen, dass jedes Implantat in die erforderliche Konfiguration gegossen oder einzeln skulpiert werden muss. Solche Implantate würden die Implantation vereinfachen und auf diese Weise komplizierte, zeitaufwändige und teuere chirurgische Verfahren limitieren. Die US-A-5496371 und die EP-A-518278, aus denen die Merkmale des Oberbegriffs des Anspruchs 1 bekannt sind, offenbaren ein Implantat mit einer Mehrzahl von zueinander beabstandeten Segmenten, die über elastische Verbindungselemente angebracht sind und die gekrümmt werden können, um die Segmente erneut anzuordnen, so dass das Implantat die ungefähre Größe und Form des zu behandelnden Körperteils annehmen kann.
- ZUSAMMENFASSUNG
- Ein Verbindungselement der vorliegenden Erfindung ist im Anspruch 1 bestimmt. Bevorzugte Ausführungsformen des Verbindungselementes sind Gegenstand der abhängigen Ansprüche 2 bis 4.
- Ein Verbindungselement für Segmente eines modularen chirurgischen Implantates ist vorgesehen, welches einen ersten Abschnitt und einen zweiten Abschnitt umfasst. Der erste Abschnitt besitzt ein erstes Ende und ein zweites Ende; das erste Ende ist zum Anbringen an ein Segment eines chirurgischen Implantates ausgebildet, und das zweite Ende besitzt eine erste Eingriffsstruktur, die passend zu einer entsprechenden Eingriffsstruktur auf dem zweiten Abschnitt ist. Der zweite Abschnitt des Verbindungselementes besitzt ein erstes Ende und ein zweites Ende; das erste Ende ist zum Anbringen eines weiteren Segmentes des modularen chirurgischen Implantates ausgebildet, und das zweite Ende enthält die zweite Eingriffsstruktur, die passend zur ersten Eingriffsstruktur des ersten Abschnittes ausgebildet ist, um so eine einrastende Schwenkverbindung zu bilden.
- Ein modulares chirurgisches Implantat der vorliegenden Erfindung ist im Anspruch 5 bestimmt. Bevorzugte Ausführungsformen des Implantates sind Gegenstand der abhängigen Ansprüche 6 und 7.
- Ein modulares chirurgisches Implantat ist vorgesehen, das zumindest ein erstes Segment und zweites Segment umfasst. Das erste Segment ist mit dem zweiten Segment über ein Verbindungselement verbunden, das einen ersten Abschnitt und einen zweiten Abschnitt besitzt. Der erste Abschnitt ist mit dem ersten Segment und der zweite Abschnitt ist mit dem zweiten Segment verbunden. Der erste Abschnitt und der zweite Abschnitt sind derart bemessen und konfiguriert, dass sie zueinander passend ausgebildet sind, und dass sie eine Schwenkverbindung derart bilden, dass das erste Segment und das zweite Segment relativ zueinander in multiplanarer Ausrichtung beweglich sind.
- Ein Verfahren zum Implantieren eines chirurgischen Implantates kann verwendet werden, so dass es das Vorsehen eines ersten und eines zweiten Segmentes eines modularen chirurgischen Implantates umfasst, wobei das erste Segment einen ersten Abschnitt eines daran angebrachten Verbindungselementes und das zweite Segment einen zweiten Abschnitt eines daran angebrachten Verbindungselementes besitzt. Der erste Abschnitt umfasst einen Eingriffsstruktur, die einer entsprechenden Eingriffsstruktur auf dem zweiten Abschnitt entspricht und mit dieser in Eingriff gebracht wird, um eine einrastende Schwenkverbindung zu bilden. Das erste Segment und das zweite Segment werden in einen Patienten eingeführt. Das erste Segment und das zweite Segment werden dadurch verbunden, indem die Eingriffsstruktur des ersten Abschnittes mit der entsprechenden Eingriffsstruktur des zweiten Abschnittes eingerastet wird. Die Form des modularen chirurgischen Implantates entspricht dadurch einer erwünschten Konfiguration, indem das erste Segment und das zweite Segment relativ zueinander geschwenkt bzw. gedreht werden. - Ein Verbindungselement für Segmente eines modularen chirurgischen Implantates und modulare chirurgische Implantate, die das hierin beschriebene Verbindungselement enthalten, ermöglichen das Austauschen standardisierter modularer Implantatsegmente verschiedener Formen und das Ausrichten mit Hilfe einer einrastenden Schwenkverbindung. Auf diese Weise können modulare chirurgische Implantate auf die Bedürfnisse einzelner Patienten ausgerichtet werden, indem bereits fertige modulare Segmente ausgewählt und aufeinander angepasst werden. Die Verwendung solch eines modularen Implantates ermöglicht es einem Chirurgen, die Einschnittgröße, die bei dem chirurgischen Eingriff vorgenommen wird, zu verkleinern, da jedes modulare Segment einzeln in den Zielbereich eingeführt und anschließend in situ zusammengesetzt werden kann. Dies steht im Gegensatz zu einem nicht-modularen, einteiligen Implantat, das einen größeren Einschnitt erforderlich macht, um so das gesamte Implantat in seiner vollen Größe einführen zu können. Die einrastende Schwenkverbindung, die eine multiplanare Ausrichtung der modularen Segmente vorsieht, ermöglicht es, dass jedes Segment auf geeignete Weise eingesetzt und unter einem Winkel oberhalb des darunter liegenden Knochens angeordnet werden kann, und dass die Form der so gebildeten Prothese, falls erwünscht, verändert werden kann.
- Ein zusammengebautes Verbindungselemente
20 ist in den1 und2 dargestellt. Das Verbindungselement20 umfasst einen ersten Abschnitt22 und einen zweiten Abschnitt24 . Der erste Abschnitt22 , der in den2 und4 getrennt gezeigt ist, umfasst ein erstes Ende26 , welches dazu dient, den ersten Abschnitt22 in ein Segment eines modularen chirurgischen Implantates zu verankern. Das erste Ende26 umfasst einen zylindrischen Korpusabschnitt28 , der an einem proximalen Ende eines Stammabschnittes30 angebracht ist. Der Stammabschnitt30 besitzt einen kleineren Umfang als der Korpusabschnitt28 . Ein distales Ende des Stammabschnittes30 ist mit einem proximalen Ende eines zylindrischen Korpusabschnittes32 verbunden. Das distale Ende des Korpusabschnittes32 ist über einen Hals34 mit einem glühbirnenförmigen Kopfabschnitt36 verbunden, welcher eine erste Eingriffsstruktur bildet, die, wie unten beschrieben, mit einer zweiten Eingriffsstruktur auf dem zweiten Abschnitt24 zusammenpasst. - Das erste Ende
26 ist derart konfiguriert, um den ersten Abschnitt22 in ein modulares Segment eines chirurgischen Implantates zu verankern. Eine solche Konfiguration kann, wie dargestellt ist, als spulenförmig beschrieben werden. Der Korpusabschnitt28 , der Stammabschnitt30 und der Korpusabschnitt32 wirken derart zusammen, um den ersten Abschnitt22 des Verbindungselementes20 zu stabilisieren, wenn dieser auf einem modularen Segment befestigt wird. Der Stammabschnitt30 sieht einen Bereich für das Material vor, das das Segment bildet, in den es fließen und gehalten werden kann, und nach dem Aushärten und/oder Verbinden wird das Verschieben des ersten Abschnittes22 aus dem Segment verhindert. Der Korpusabschnitt28 und der Korpusabschnitt32 stabilisieren den ersten Abschnitt22 , indem sie miteinander zusammenwirken, um so eine radiale Verschiebung des ersten Abschnittes22 zu verhindern. - Der zweite Abschnitt
24 des Verbindungselementes20 , der in den2 und5 getrennt gezeigt ist, umfasst ein erstes Ende38 , das, ähnlich dem ersten Ende des ersten Abschnittes, dazu dient, den zweiten Abschnitt24 in ein weiteres Segment eines modularen chirurgischen Implantates zu verankern. Das erste Ende38 umfasst einen zylindrischen Korpusabschnitt40 , der an einem proximalen Ende eines Stammabschnittes42 angebracht ist. Der Stammabschnitt42 besitzt einen kleineren Umfang als der Korpusabschnitt40 . Ein distales Ende des Stammabschnittes42 ist mit einem proximalen Ende eines zylindrischen Korpusabschnittes44 verbunden. - Das erste Ende
38 des zweiten Abschnittes24 ist derart konfiguriert, um den zweiten Abschnitt in ein modulares Segment eines chirurgischen Implantates zu verankern. Eine solche Konfiguration kann, wie dargestellt ist, als spulenförmig beschrieben werden. Der Korpusabschnitt40 , der Stammabschnitt42 und der Korpusabschnitt44 wirken derart zusammen, um den zweiten Abschnitt24 zu stabilisieren, wenn dieser auf einem modularen Segment befestigt wird. Der Stammabschnitt42 sieht einen Bereich für das Material vor, das das Segment bildet, und in den es fließen und gehalten werden kann, und zwar nach dem Aushärten oder Binden, und wodurch ein Verschieben des zweiten Abschnittes24 aus dem Segment verhindert wird. Der Korpusabschnitt40 und der Korpusabschnitt44 stabilisieren den zweiten Abschnitt24 , indem sie miteinander zusammenwirken, um eine radiale Verschiebung des zweiten Abschnittes24 zu verhindern. - Das zweite Ende
46 des zweiten Abschnittes24 enthält eine zweite Eingriffsstruktur, die der ersten Eingriffsstruktur des ersten Abschnittes22 entspricht, um so eine einrastende Schwenkverbindung vorzusehen. Das zweite Ende46 des zweiten Abschnittes24 umfasst eine Aufnahme48 mit Expansionsschlitzen50 und52 . Das offene Ende oder der Rand der Aufnahme48 ist in radialer Richtung nach innen gekrimpt, und die Aufnahme48 ist derart konfiguriert, dass sie den glühbirnenförmigen Kopf36 des ersten Abschnittes22 aufnehmen und darin halten kann. Falls der erste Abschnitt22 mit dem zweiten Abschnitt24 zusammengepasst bzw. zusammengefügt wird, wird der Kopf durch die gekrimpte Öffnung in die Aufnahme48 geschoben. Die Expansionsschlitze50 und52 ermöglichen es, dass sich die Aufnahmewände nach außen bewegen, während der Kopf36 die gekrimpte Öffnung auseinander drückt. Sobald der breiteste Abschnitt des Kopfs sich hinter dem gekrimpten Abschnitt bewegt, rastet bzw. schnappt die Öffnung der Aufnahme48 in ihren ursprünglichen Durchmesser und hält so den Kopf36 . Bei einer bevorzugten Ausführungsform passt der Kopf36 auf feste und sichere Weise in die Aufnahme48 , ist aber nicht daran gehindert, sich in der Aufnahme zu drehen. Eine Schnapppassung sieht einen gewissen Widerstand gegenüber einer Reibungsbewegung und einer nicht gewollten weiteren Bewegung des Verbindungselementes20 vor, wenn einmal eine erwünschte Ausrichtung des ersten Abschnittes22 und des zweiten Abschnittes24 erzielt ist. Die Drehung bzw. Schwenkbewegung wird lediglich dann eingeschränkt, wenn der Hals24 in Kontakt mit den Wänden der Aufnahme48 tritt. - Das Verbindungselement
20 kann aus einer Vielzahl von Materialien hergestellt sein. Optimale Materialien sind biokompatibel und ausreichend fest, um einer im Rahmen liegenden Spannung, die dann erzeugt wird, wenn der erste Abschnitt22 und der zweite Abschnitt24 zusammenschnappen, und einer im Rahmen liegenden Spannung, die im Zusammenhang mit dem Ausrichten der Segmente eines modularen chirurgischen Implantates steht, zu widerstehen. Bevorzugte Materialien sind solche Metalle, wie z. B. rostfreier Stahl und Titan. Polymermaterialien, wie z. B. Derivate, wie z. B. Karbonate, Terephthalate, Olefine, Amide und Urethane können eingesetzt werden. Im Falle von Metallen kann der erste Abschnitt22 und der zweite Abschnitt24 gegossen, gestanzt oder maschinell bearbeitet sein, um so die geeignete Formen zu erzielen. Die Polymermaterialien können ebenso in geeignete Formen gegossen, gestanzt oder maschinell bearbeitet sein. Es ist in Betracht gezogen worden, dass beliebige Formverfahren, die dem Fachmann bekannt sind, eingesetzt werden können, um das Verbindungselement20 zu bilden. - Das Verbindungselement
20 ermöglicht es, dass eine Mehrzahl modularer Segmente zusammengesetzt werden können zur Erzeugung kundenspezifischer Implantate aus massenproduziertem Lagermaterial. Maxillofaziale Implantate sind besonders für die Anwendung der vorliegenden Offenbarung geeignet. Z. B. ist das modulare Kinnimplantat60 , das in den6 bis9 dargestellt ist, derart konfiguriert, den lateralen (externen), äußeren (vorderen) Abschnitt der Mandibula, wie in10 gezeigt ist, zu behandeln. Das Kinnimplantat60 umfasst drei modulare Segmente: ein erstes Kinnsegment62 , ein zweites Kinnsegment64 und ein drittes Kinnsegment66 . Der zweite Abschnitt24 ist an dem ersten Kinnsegment62 angebracht, d. h. das erste Ende38 (gezeigt in gestrichelten Linien) verankert den zweiten Abschnitt24 in dem ersten Kinnsegment62 . Der Korpusabschnitt40 des zweiten Abschnittes24 erstreckt sich von dem ersten Kinnsegment62 nach außen, wo das zweite Ende46 offen ist, um so mit einem ersten Abschnitt22 (6 ) verbunden zu werden. - Die beiden Abschnitte
22 werden an gegenüberliegenden Seiten des zweiten Kinnsegmentes64 angebracht. In beiden Fällen verankert das erste Ende26 (gezeigt in gestrichelten Linien) den ersten Abschnitt22 in dem zweiten Kinnsegment64 . Die Körperabschnitte32 der ersten Abschnitte22 erstrecken sich von beiden Seiten des zweiten Kinnsegmentes64 nach außen, wo die Kopfabschnitte36 nach außen vorstehen, um so in Eingriff mit einem beliebig geeigneten zweiten Abschnitt24 zu treten. Ein weiterer zweiter Abschnitt24 wird an dem dritten Kinnsegment66 ähnlich wie der zweite Abschnitt24 an dem ersten Kinnsegment62 angebracht, d. h., das erste Ende38 , das in dem dritten Kinnsegment66 enthalten ist, und das zweite Ende46 , das nach außen hervorsteht, um den Kopf36 einer ersten Eingriffsstruktur aufzunehmen. Das modulare Kinnimplantat60 ist in7 zusammengesetzt gezeigt, wobei die erste Eingriffsstruktur des ersten Abschnittes22 mit der zweiten Eingriffsstruktur des zweiten Abschnittes24 zusammengepasst ist, um eine drehbare Schnappverbindung bzw. eine einrastende Schwenkverbindung zu bilden. - Entsprechend der Funktionsweise des Verbindungselementes
20 können die modularen Segmente62 ,64 und66 unter einem beliebigen Winkel innerhalb des Schwenkradius, der durch die einrastende Schwenkverbindung vorgesehen ist, ausgerichtet werden, um so eine Einstellung des Implantates60 zu ermöglichen, die einer speziellen Knochenstruktur eines Patienten entspricht. Auf diese Weise können nicht nur unterschiedlich große Segmente und Segmente mit unterschiedlichen Formen ausgetauscht werden, sondern auch die Winkel, unter denen die Segmente des Implantates die darunter liegende Knochenstruktur schneiden, können verändert werden. Um z. B. ein Kinn mit einem kleineren Krümmungsradius unterbringen zu können, können die Segmente62 und66 nach innen in bezug auf das Kinnsegment zu der in8 gezeigten Konfiguration bewegt werden, wie dies durch die Pfeile angezeigt ist. Die Verbindungselemente20 ermöglichen eine Konfiguration der Segmente derart, dass eines der Segmente62 ,64 und/oder66 nach innen, nach außen, nach oben, nach unten, in diagonaler Richtung oder in eine beliebige Richtung relativ zueinander bewegt werden können. Siehe9 . Überdies ermöglicht die Schwenkverbindung, dass die Segmente koaxial in bezug auf ein benachbartes Segment gedreht werden können. - Das hierin beschriebene Verbindungselement
20 kann dazu verwendet werden, Segmente eines maxillofazialen modularen chirurgischen Implantates, einschließlich von Kinn-, Jochbein-, paranasalen, angulus mandibulae und prämaxillären Implantaten, zu verbinden. Solche Implantate sind in den10 und11 dargestellt, wo sie an einen menschlichen Schädel S angebracht sind. Die rechte und linke Seite der modularen Jochbein-Implantate70 und72 sind derart positioniert, dass sie den seitlichen Abschnitt des rechten bzw. linken Jochbeins, wie in10 gezeigt ist, verstärken oder ersetzen. Die rechte und linke Seite der modularen paranasalen Implantate74 und76 sind derart positioniert, um den seitlichen Abschnitt der Maxilla entlang der linken und rechten Seite der inferioren nasalen Concha zu behandeln. Das modulare Kinnimplantat60 ist ebenso im befestigten Zustand an dem Kinnknochen in10 gezeigt. Ein modulares, linksseitiges Unterkiefer-Implantat78 und ein modulares, prämaxilläres Implantat sind im eingesetzten Zustand am Schädel S in11 gezeigt. - Es ist ebenso in Betracht gezogen worden, dass das hierin beschriebene Verbindungselement
20 nicht auf die Verwendung zum Verbinden modularer Segmente von Kinn-, Jochbein-, paranasalen, angulus mandibulae oder prämaxillären Implantaten begrenzt ist, sondern dass es auch bei modularen Implantaten für einen beliebigen Teil des Körpers verwendet werden kann. - Die modularen Segmente des Implantates können aus einem beliebigen biokompatiblen Material hergestellt sein, das einer im Rahmen liegenden Spannung widerstehen kann, denen die Segmente während oder nach der Implantation ausgesetzt sind. Die Segmente sollten fest und von langer Lebensdauer sein, und in manchen Fällen können sie derart ausgestaltet sein, dass sie auf wirkungsvolle Weise das Wachstum von Gewebe oder Knochen in oder um ihre Position induzieren können. Bei einer bevorzugten Ausführungsform ist eine poröse Struktur einer Polymerzusammensetzung, wie z. B. Polymethylmethacrylat (PMMA), mit einem polymerischen hydrophilen Material, wie z. B. Polyhydroxyethylmethacrylat (PHEMA), beschichtet. Verschiedene Materialien, die die Segmente für Röntgenstrahlen undurchlässig machen, können ebenso enthalten sein, z. B. Bariumsulfat. Diese Materialien und die Verfahren zu deren Herstellung sind in den US Patenten Nr. 4,547,390, 4,547,327 und 4,728,570 offenbart. Es ist ebenso in Betracht gezogen worden, dass ein anderes geeignetes Prothesenmaterial, das dem Fachmann bekannt ist, wie z. B. dichte oder poröse Keramiken, Polymer/Keramik Verbundstoffe oder Metalle, entsprechend der vorliegenden Offenbarung jedoch verwendet werden kann. Diese Materialien können ebenso z. B. resorbierbare Polymermaterialien umfassen, wie z. B. polymere oder co-polymere des Glyokolid oder Laktid oder Polydioxanon.
- Beim Formen der modularen Implantate kann der Abstand zwischen benachbarten Segmenten verändert werden, um unterschiedlichen Anwendungen gerecht zu werden, indem die Länge des Verbindungselementes
20 vergrößert oder verkleinert wird. Ein größerer Abstand zwischen den Segmenten ermöglicht einen größeren Winkel zwischen benachbarten Segmenten. Der Abstand zwischen den Segmenten liegt vorzugsweise im Bereich von 0,13 mm bis 6,35 mm (0,005 inch bis 0,25 inch); größere oder kleinere Abstände liegen jedoch innerhalb des Bereichs der vorliegenden Erfindung. In Situationen, in denen die Segmente weit voneinander beabstandet sind, können die Lücken zwischen den Segmenten mit einem Knochenersatz gefüllt werden, wie z. B. einem granularen HTR® Polymer, eine Marke der United States Surgical Corporation. Überdies kann eine unterschiedliche Anzahl von Segmenten, als dargestellt, verwendet werden (z. B. eine größere Anzahl von Segmenten zum Erhöhen der Flexibilität), und die Form und Konfiguration der einzelnen Segmente (z. B. die Position der Segmentation) kann abhängig von seiner Verwendung verändert werden. - Obwohl das Kinnimplantat
60 , die paranasalen Implantate74 und76 , die Jochbein-Implantate70 und72 , das angulus mandibulae Implantat78 und das prämaxilläre Implantat80 so dargestellt sind, als ob sich Lücken in Längsrichtung zwischen den Segmenten befänden, so ist ebenso in Betracht gezogen worden, dass diese Lücken in Breitenrichtung oder diagonal verlaufen. In der Tat können in Breitenrichtung, Längsrichtung und Diagonalrichtung verlaufende Lücken in dem selben modularen Implantat enthalten sein, um so das Anordnen der Segmente unter beliebigen geeigneten Winkeln und Richtungen zu ermöglichen. Es ist ebenso in Betracht gezogen worden, dass mehr als nur ein Verbindungselement verwendet werden kann, um die Lücke an verschiedenen Punkten zwischen Segmenten zu überwinden. - Der Fachmann verwendet ein geeignetes Verfahren oder Mittel, das bekannt ist, zum Anbringen der hierin beschriebenen modularen chirurgischen Implantate an die Gesichtsknochen des Schädels. Z. B. können Metall- oder Plastikschrauben durch eines oder mehrere der Segmente des Implantatmaterials eingeführt werden, um das Implantat am Körper anzubringen.
- Es ist offensichtlich, dass verschiedene Modifikationen an den hierin beschriebenen Ausführungsformen vorgenommen werden können. Z. B. ist das Verbindungselement
20 hier meistens zylindrisch dargestellt, es ist aber ebenso in Betracht gezogen worden, dass es einen rechtwinkligen, polygonalen, quadratischen, ellipsoiden etc. Querschnitt besitzen kann. Der Kopf ist als glühbirnenförmig dargestellt, kann aber ebenso andere Konfigurationen, wie z. B. polygonal oder dergleichen besitzen. Der Verankerungsabschnitt des Verbindungselementes ist als spulenförmig beschrieben, kann aber ebenso andere unregelmäßige Formen besitzen, so lange die Ankerfunktion erfüllt wird. Falls mehr als nur ein Verbindungselement in ein Segment eines modularen chirurgischen Implantates eingesetzt wird, z. B. wie in6 bis10 (Segment64 ) dargestellt ist, können zweite Abschnitte anstelle der ersten Abschnitte verwendet werden, oder eine Seite des Segmentes kann einen ersten Abschnitt enthalten, während die andere Seite einen zweiten Abschnitt enthalten kann, so lange wie entsprechende Eingriffsstrukturen zur Verfügung stehen, die mit dem jeweils benachbarten Segment zusammenpassen. Deshalb sollte die obige Beschreibung nicht als limitierend verstanden werden, sondern lediglich als beispielhafte Darstellung bevorzugter Ausführungsformen. - Die im Anschluss folgende Ansprüche identifizieren Ausführungsformen der Erfindung zusätzlich zu denjenigen, die im Detail bereits beschrieben wurden.
Claims (7)
- Verbindungselement (
20 ) für Segmente eines modularen chirurgischen Implantats (60 ), wobei das Verbindungselement einen ersten Abschnitt (22 ) und einen zweiten Abschnitt (24 ) aufweist, wobei der erste Abschnitt ein erste Ende (26 ) und ein zweites Ende aufweist, wobei das erste Ende eine Verankerung definiert, durch die das erste Ende an einem Segment eines modularen chirurgischen Implantats befestigt werden kann, wobei der zweite Abschnitt ein erstes Ende (38 ) und ein zweites Ende (46 ) aufweist, wobei das erste Ende eine Verankerung definiert, durch die das erste Ende des zweiten Abschnitts an einem anderen Segment des modularen chirurgischen Implantats befestigt werden kann, gekennzeichnet dadurch, dass die jeweiligen zweiten Enden des ersten und zweiten Abschnitts beide jeweilige Eingriffsstrukturen (36 ,48 ) einbeziehen, die zueinander passend gestaltet sind, um eine einrastende Schwenkverbindung zwischen dem ersten und zweiten Abschnitt zu bilden, so dass der erste und zweite Abschnitt relativ zueinander in multiplanarer Ausrichtung beweglich sind. - Verbindungselement für Segmente eines modularen chirurgischen Implantats nach Anspruch 1, wobei die erste Eingriffsstruktur ein glühbirnenförmiges Element (
36 ) ist, und die zweite Eingriffsstruktur eine Buchse (48 ) ist, die gestaltet ist, um das glühbirnenförmige Element aufzunehmen und festzuhalten. - Verbindungselement nach Anspruch 1 oder 2, wobei das erste Ende des ersten Abschnitts spulenförmig ist.
- Verbindungselement nach Anspruch 1, 2 oder 3, wobei das erste Ende des zweiten Abschnitts spulenförmig ist.
- Verbindungselement nach einem der vorstehenden Ansprüche, das in einem modularen chirurgischen Implantat mit zumindest einem ersten Segment (
62 ) und einem zweiten Segment (64 ) einbezogen ist, wobei das erste Segment mit dem zweiten Segment über das Verbindungselement verbunden ist, wobei der erste Abschnitt an dem ersten Segment befestigt ist, und der zweite Abschnitt an dem zweiten Segment befestigt ist. - Modulares chirurgisches Implantat nach Anspruch 5, wobei das modulare chirurgische Implantat dimensioniert und gestaltet ist, so dass es zumindest einem Teil eines maxillofacialen Knochens angepasst ist.
- Modulares chirurgisches Implantat nach Anspruch 5 oder 6, wobei das erste und das zweite Segment zumindest teilweise biologisch absorbierbar ist.
