Abstract

The method for preparing and dispensing a mixture of two components, in particular a bone cement of low viscosity, is simplified and improved in that the components are stored at the required volume ratio in a cartridge divided into two chambers by a partition (3), in that the partition (3) is then broken by external influence on the cartridge and the latter is vigorously shaken back and forth, the components contained therein being mixed, and in that finally the mixture is squeezed out of the cartridge. The cartridge has a squeezing-out plunger (5) and an internal sterile packaging (13, 14, 15). A plurality of cartridges are stored at an optimum temperature in a central unit which also contains a movement device for the cartridge. After moving/mixing, the cartridge is opened by turning the lid (14) of the package and is then subjected to a vacuum. To discharge the mixture, the cartridge is finally inserted in a hand-piece whose push rod presses against the discharging plunger (5) (see diagram). <IMAGE>

Description

The invention relates to a method for preparing and Spreading a mixture of two components, especially a low-viscosity bone cement as well Devices for performing the method.

A preferred field of application of the invention is Processing of plastics on the basis of Methyl methacrylate in clinical medicine. she is but not limited to this.

It was used to fix hip endoprostheses Bone cement material used to be made exclusively by hand applied. Recent developments have led to Bone cements based on polymethyl methacrylate (PMMA) using a syringe in the bones be introduced. First, the Monomer liquid and the polymer powder by hand mixed, resulting in a low-viscosity mass the air bubbles during a certain idle time can escape. The mass is then in a syringe filled and must then within a certain period of for example four minutes.

However, this procedure is cumbersome. The mixing of components required even when using a Mixer a considerable handiwork and relatively much time; as well as decanting the Mix into the syringe. In both processes there is Risk of contamination, which is why special precautions must be taken to ensure sterility.

The invention has for its object a method to propose to the surgeon with easier handling  a bone cement of the highest quality Permitted to provide, this quality human influence and thus also human Failure is largely eluded.

This task is based on a process of introductively designated type according to the invention solved that the components in the required Volume ratio in one by a partition in two Chambers shared cartridge are kept that then the partition by acting on the cartridge from the outside pierced and the cartridge moved violently back and forth is, the components contained therein mix, and that finally the mixture from the cartridge is squeezed out. The simplification is obvious there Storage, transport, mixing and processing of the Plastic mass in or out of the same container he follows. The risk of contamination when mixing and when Pouring into a syringe is not necessary. With the removal of the Pouring around there is also no resulting Air pockets more. The mixing process is extreme quickly and the result is a homogeneous mixture of two components. The extended stance phase results in better prepolymerization or Secondary cross-linking of the macromolecules and, as a result, one better mechanical quality of the plastic.

There is an essential further development of the method in that the cartridge before mixing on a certain, of the type of plastic and the Application depending on the desired temperature brought and after mixing with an evacuation opening provided and subjected to a vacuum. Through the The processing time can be pre-selected for the temperature influence; for example, extends a deeper one Mixing temperature the processing time. By appropriate  The process is suitable for temperature adjustment different plastics with different phase times. Since that Mix extremely little time by shaking the cartridge requires and the transfer into a syringe is completely eliminated, can use the entire service life as an evacuation phase with the success that the resulting from mixing Air bubbles are more completely removed from the crowd and no pores remain in the hardened material its mechanical strength would weaken.

There is a cartridge to perform this procedure expediently removed from a tubular body with a or breakable radial partition, a first Front wall with a closure for making one Discharge opening and one instead of the opposite second end wall inserted into the tubular body Ejection piston. Experience has shown that they mix with the liquid and powdery example mentioned in the introduction Component only by shaking the cartridge when the Partition is broken. The latter can achieved that a relatively rigid partition Has predetermined breaking lines and the tube body so elastic is that the partition by transverse compression of the Pipe body can be broken. In addition, there is Possibility of one in one of the two chambers Mixing body, for example a specifically heavy ball accommodate the partition when shaking penetrates and also a thorough mixture favored. Finally, the tubular body can be in two parts be formed, the two parts move or rotate mutually limited telescopically can, and this relative movement to destroy the Partition is used.

The cartridge is preferably sterile with a first one Provide packaging that is designed so that it  allowed without the use of tools, with a simple movement on the cartridge and the packaging without compromising sterility evacuation openings to attach, so that in the resting phase after mixing the cartridge opened in this way a vacuum can be subjected to the rise of the Air bubbles accelerate. For example, at the Packaging a mandrel with which this and the Cartridge can be pierced or the openings could be created by tearing out wall pieces will. It is particularly useful on the packaging and to attach the screw caps to the cartridge are coupled together and an evacuation channel to Release the inside of the cartridge. It can be used as a rotary, Sliding or zipper closure trained the cartridge at the same time to create a Ejection opening for the mixed cartridge content serve.

It is necessary to carry out the method according to claim 1 further a handpiece, which a holder in which the Cartridge can be used, and one on the Ejection piston of the cartridge acting press device having. The pressing device can be a hand too actuating plunger. In case of need the mixed plastic through a fine cannula can eject, or if the viscosity is higher the hand tappet also by a power assisted Press device to be replaced, the necessary for this Auxiliary device, for example pump, still in the bottom descriptive central unit can be accommodated. A preferred embodiment of the handpiece exists in that the bracket is pivoted sideways Has guide part in which the on Ejection piston of the cartridge acting plunger is guided and through which the cartridge in the bracket can be locked.

In order to ensure that the to achieve both components and the same mixing times it is proposed that a mechanical mixing device is provided which has a rotary disc Drive motor and one on the rotating disc cartridge carrier rotatably mounted in parallel axis, the same when rotating the rotary disc Alignment is held. The latter can be done by means of a movable linkage, an elastic fixation or can be achieved by geared means, always with the Aim to move the cartridge lengthways and crossways move.

All devices described can be in one so-called central unit for performing the method be summarized. This consists of a housing that preferably at least one storage compartment for storage for a plurality of cartridges and the mixing device having. The storage compartment should have a thermostatic adjustable cooling and / or heating device, to the desired optimal temperature for mixing and Preselect processing. An advantageous one Training of the central unit can consist in that a temperature sensor is provided for the room temperature and an electronic calculator that records processing time of the mixture depending on the room temperature determined and displayed.

The cartridge shown in Figs. 1 to 6 consists of a tubular body ( 1 ) with two opposing cams ( 2 ) attached to the outer circumference. The tubular body has a radial partition ( 3 ) with predetermined breaking points ( 4 ) in the shape of a groove along the edge and the surface in a cross shape into four parts, in the area of which the wall thickness is reduced to a small fraction. At the lower end, an elastic ejection piston ( 5 ) is inserted in the tubular body, which has a stiffening plate ( 6 ) and is covered with a film ( 7 ) which is tightly welded to the end edges of the tubular body.

The opposite upper end wall ( 8 ) of the tubular body ( 1 ) has two sector-shaped openings ( 9 ), which are smaller than the fragments formed when the partition wall ( 3 ) is destroyed, so that they cannot be ejected under any circumstances. On this upper end of the tubular body, a cap ( 10 ) is rotatably placed, which has a cylindrical and a funnel-shaped part and a radial transverse wall. In the transverse wall two sector-shaped openings ( 11 ) are also provided, which are in the angular position of the cap shown in Fig. 3 between the openings ( 9 ), so that the cartridge is closed. The funnel of the cap ( 10 ) is deformed into a mouthpiece with an elongated opening ( 12 ).

The cartridge is in a sterile packaging, which essentially consists of a central part ( 13 ) with a lid ( 14 ) made of a dimensionally stable plastic and a film ( 15 ). The middle part ( 13 ) has two pockets ( 16 ) for receiving the cams ( 2 ) of the cartridge and an upper cylindrical section on which the cover ( 14 ) is rotatably placed. Rectangular recesses ( 17 ) and ( 18 ) are provided in the cylinder jackets of the middle part and the cover, which, when they are one above the other, represent openings in the packaging. In the middle, the cover (14) to below the fingers (19) which reach into the opening (12) of the cap hopper, and are arranged so that by rotation of the lid (14) by 90 °, the closures (8, 10 or 13 , 14 ) of the cartridge and the packaging are actuated together in the same direction.

The film ( 15 ) as part of the mentioned first sterile packaging envelops the lower part of the cartridge. A further second or outer sterile packaging, indicated by the rectangle ( 20 ), is provided for storage and transportation.

The central unit shown in FIG. 7 is designed as a tabletop device and contains in its rear part a temperature-controlled storage compartment for several cartridges, the hinged lid of which is identified by ( 21 ). A mixing device is located in the middle section of the housing and is accessible via a cover ( 22 ). The front part of the housing ( 23 ) contains the electrical control and regulating devices and a control panel ( 24 ). The required connecting lines are indicated with ( 25 ).

FIG. 8 shows a preferred embodiment of a handpiece, consisting of an essentially cylindrical holder ( 26 ) with lateral openings ( 27 ) and with a guide part ( 28 ) which extends around an axis ( 29 ) running radially to the holder ( 26 ). is pivotable. A hand tappet ( 30 ) is mounted for longitudinal displacement in the guide part. It has a widened head ( 31 ) at the inner end, which rests on the ejection piston ( 5 ) of the inserted cartridge.

The mixing device indicated in FIGS. 9 and 10, which is installed in the housing of the central unit shown in FIG. 7, essentially consists of a horizontally axially mounted rotating disc ( 32 ) and a cartridge carrier ( 33 ) which is attached to the rotating disc about an axis ( 34 ). is rotatable, which runs parallel to the axis of the rotary disc. The cartridge carrier ( 33 ) is shaped such that it can be inserted with its inner packaging after the outer packaging ( 20 ) has been removed and can be fastened accordingly. In order to prevent the cartridge carrier ( 33 ) from rotating in space and thus to cause the cartridge to move approximately parallel to itself, the cartridge carrier is fastened to the housing by means of two elastic strands ( 35 ). To achieve a circular movement of the cartridge that is exactly parallel to itself, two rotary disks connected to one another by a crank rod could also be provided, the cartridge carrier having to be attached to the crank rod.

In the example, the upper, larger chamber of the cartridge according to FIG. 1 is filled with the powdery component and the lower chamber with the liquid component of a bone cement based on polymethyl methacrylate. Before mixing, the partition ( 3 ) is destroyed by pressing the tube body ( 1 ) by hand in the direction of the arrows ( 37 ). However, a dash-dotted ball ( 36 ) can also be provided in one of the chambers, which penetrates the partition ( 3 ) when the cartridge is shaken and also promotes the mixing process.

The device described works as follows: The daily requirement for bone cement according to the operation program is accommodated in the thermal compartment of the central unit. The Zudi sister takes a cartridge from this supply and removes its outer packaging ( 20 ). It then opens the mixing compartment cover ( 22 ) and inserts the cartridge with the inner packaging into the cartridge holder ( 33 ). Possibly. it breaks the partition beforehand by squeezing the lower part of the cartridge. After closing the lid ( 22 ), the drive motor of the rotary disc ( 32 ) is started up in accordance with the specification of the bone cement in question and automatically stopped again after the mixing process has ended. This is indicated optically and acoustically. The student now opens the lid ( 22 ) again, turns the lid ( 14 ) of the outer packaging by 90 °, switches on the vacuum pump and closes the lid again. This process could also be carried out automatically while accepting the necessary technical effort. The end of the evacuation phase / standing phase is also indicated optically and acoustically. The point in time depends on the room temperature and is determined by the computer installed in the central unit specifically for the bone cement used. Now the cartridge with the inner packaging is removed from the cartridge carrier ( 33 ) and the lid ( 14 ) of the packaging is removed. The now accessible cartridge is then removed from the inner packaging by the surgical nurse working with sterile gloves and appropriate clothing and inserted with the cap ( 10 ) ahead into the holder ( 26 ) of the sterile handpiece. After swiveling back the guide part ( 28 ) with the plunger ( 30 ) completely pulled out, the cartridge is locked in the holder ( 26 ) and is now ready to be sprayed out by the surgeon at exactly the right time. The end of the processing phase, which is dependent on the room temperature, can also be indicated optically and acoustically. The characteristic data of the bone cement used can be entered on the control panel ( 24 ), so that the temporal process sequence is always optimally controlled even with a different chemical composition.
