技术领域
本发明总体涉及脊柱植入物和假体(prostheses),尤其涉及具有术后可调节尺寸的脊柱植入物。
背景技术
椎管狭窄,以及椎关节强硬,脊椎前移,骨关节炎,脊柱侧凸及其它变性现象可以是背部疼痛的原因,并且可以由对脊髓和/或神经根产生压迫的脊柱管或孔(foramina)的窄化所引起。
用于解决背部疼痛的方法之一包括减压,切除引起疼痛的骨头突出部分(bony element),以及融合两个或多个相邻脊椎。不幸地,融合倾向于具有显著的缺陷并可引起移动(migrate)到相邻脊椎部分(component)的问题。在非融合中,解决方案是盘替换,动态固定系统和棘突间(inter-spinousprocess)植入物。
具有高度调节能力的脊柱植入物是已知。例如,Hochshuler等人的美国专利6045579,6080193和6576016(分别在2000年4月4日,2000年6月27日和2003年6月10日公布)描述了一种用于促进相邻脊椎之间的脊柱融合的可调节高度的融合装置。该装置位于椎间盘空间内并包括一对用于接触脊椎的接合盘。对准装置用于改变接合盘之间的垂直距离以定制适合指定病人的设备。在一个实施例中,对准装置包括一对具有预定高度并在接合盘之间从设备的前端延伸至后端的支柱(strut)。在另一个实施例中,对准装置包括可旋转的连接器和用于调节接合盘之间距离的凸轮销。对准装置优选适于改变接合盘之间的距离,使得接近前端的设备高度大于接近后端的高度,由此在安装设备之后保持脊柱的自然脊柱前凸。
可是,这些现有的装置必须在安装之前或安装期间进行调节而不能术后调节。
发明内容
本发明设法提供一种具有术后可调节尺寸的改进脊柱植入物(或假体,术语是可互换使用的),如棘突间脊柱植入物,以下将进一步详细描述。
在一个实施例中,通过遥控或根据与植入物的可调节部分相连的机械特征,如通过软组织中的小穿刺,脊柱植入物的至少一个尺寸在植入后能够被修改。在一个实例中,可调节部分(还称作可变尺寸机构)能够具有借助小压力管进行液压或气压激活的象活塞似的构造。可使用或配置其它的机械装置,诸如但不限于杆、缆(cable)或其它机械特征。在另一实例中,可调节部分可以是电动的,如借助电动机(如通过电线,电池或远程感应供电),并且经由遥控进行控制。
假体被构造成两个脊椎——更优选地但不限于相邻脊椎——之间的桥。假体包括构造以与多个骨连接点,诸如但不限于棘突,脊椎端盘或椎弓根(pedicles)(通过椎弓根螺钉(pedicle screws))相连的多个连接部件(端部特征)。
因此,根据本发明的非限定性实施例提供一种脊柱植入物,包括:用于连接至第一脊柱部分的第一脊柱连接件;用于连接至第二脊柱部分的第二脊柱连接件;以及植入后可变尺寸装置,设置于所述第一和第二脊柱连接件之间,在完成将所述脊柱植入物安装在病人体内的手术之后该植入后可变尺寸装置是可操作的,以导致所述第一和第二脊柱连接件之间的相对移动。
根据本发明的实施例,第一和第二脊柱连接件包括椎弓根螺钉。
根据本发明的实施例,植入后可变尺寸装置改变第一和第二脊柱连接件之间的距离。

具体实施方式
现参照图1,其说明了根据本发明实施例进行构造和操作的脊柱植入物10。
所示脊柱植入物10被植入在腰脊柱的两个相邻棘突之间(在该实例中,脊柱植入物10为棘突间固定装置)。脊柱植入物10包括置于第一(上)支撑端板(脊柱连接部件)14和第二(下)支撑端板(脊柱连接部件)16之间的植入后可变尺寸装置12。植入后可变尺寸装置12可包括柱,其被设置适于如借助被远程控制的微型线性致动器进行线性移动(根据图从垂直方向略微倾斜)。通常,植入后可变尺寸装置12可以根据以下参照图6-9描述的任意实施例进行构造。
现参照图2,其说明了根据本发明另一实施例进行构造和操作的脊柱植入物20。脊柱植入物20包括植入后可变尺寸装置22,并与椎弓根螺钉24(脊柱连接部件24)相连。可变尺寸装置22的活动改变螺钉24之间的距离。在此,植入后可变尺寸装置22还可包括柱,其被设置适于如借助被远程控制的微型线性致动器进行线性移动。通常,植入后可变尺寸装置12可以根据以下参照图6-9描述的任意实施例进行构造。

脊柱植入物40包括分别支撑上级棘突41和下级棘突43第一和第二支撑板(脊柱连接部件)44和46。第一和第二支撑板44和46各自攻有(tapped with)螺纹孔以便接收螺纹杆45L和45R以及47L和47R。螺纹杆45L和47L具有左旋螺纹,而螺纹杆45R和47R具有右旋螺纹。
齿轮滑轮(gear pulley)48A和48B与螺纹杆45L、45R、47L和47R相连,并由蜗轮49驱动。蜗轮49的旋转改变第一和第二支撑板44和46与所支撑的棘突之间的距离。
现参照图5,其说明了包括根据本发明又一实施例进行构造和操作的植入后可变尺寸装置52的脊柱植入物50。
脊柱植入物50可用于改变相邻脊椎51和53的位置。脊柱植入物50包括具有螺纹槽第一(上)支撑板54,螺纹螺杆55被螺旋地接收在螺纹槽中。脊柱植入物50包括第二(下)支撑板56,其包括其中配置有电动机(或致动器)57的凹口。电动机57(可被遥控)使螺杆55旋转,这引起第一支撑板54相对于第二支撑板56滑动。第一和第二支撑板(脊柱连接部件)54和56之间的倾斜配合在垂直面和矢状面上引起两个脊椎之间相邻位置的改变。

现参照图8,其说明了根据本发明实施例进行构造和操作的电操作的植入后可变尺寸装置80。可变尺寸装置80包括与电活塞(电致动器或螺线管)84枢轴地连接并由该电活塞移动的铰链臂82,所述电活塞可由感应线圈86外部操作。
电操作的植入后可变尺寸装置80可具有内部植入部分和/或外部控制部分。内部植入部分可以但非限制地包括电活塞84,电动机,微处理器,RF发射器/发送器,LVDT(线性可变差分变换器),应变传感器,用于进行从外部的线圈到电动机的直接能量转换的电气线圈,电池,蓄能电容器,或其任意组合。

现参照图10,其说明了根据本发明实施例进行构造和操作的植入后可变尺寸装置,其可被放置在两个相邻的棘突之间。可变尺寸装置包括第一支撑元件95和第二支撑元件96。两个两侧的相同螺纹(相同旋转方向)螺杆97和98被用于控制支撑元件95和96之间的距离。

现参照图13,其说明了根据本发明另一实施例进行构造和操作的植入后可变尺寸装置,其可被放置在两个相邻的棘突之间。该可变尺寸装置包括第一支撑元件107和第二支撑元件108。两个两侧的相同螺纹的螺杆110用于控制支撑元件107和108之间的距离。升降螺杆由电动机109激活的齿轮系所驱动。
由此,在图10-13的实施例中,脊柱植入物位于两个相邻棘突之间,使得下级棘突由植入物的第一元件所支撑以及上级棘突由植入物的第二元件所支撑。两个支撑元件沿基本水平面彼此分开,并且机械或电机机构能够控制两个支撑元件之间的距离。
图13中示出了本发明任意实施例的另一选项(option)。脊柱植入物可包括处理器120和传感器122,该处理器120和传感器122将植入后可变尺寸装置作为第一和第二脊柱连接件上所施加的负荷或第一和第二脊柱连接件之间的距离的函数来控制。植入后可变尺寸装置是可编程的以便根据任意所需负荷和行程限制以任意所需时间间隔激活。对于保存能量,植入后可变尺寸装置包括适于以睡眠模式进行操作的电路124。植入后可变尺寸装置可由可充电电池126提供能量,该电池能够利用远程能量转换器进行充电。
本发明还用于包括前进中的调节的脊柱侧凸修正,不需要重复的手术介入或融合。
例如,现参照图14,其说明了包括根据本发明另一实施例进行构造和操作的植入后可变尺寸装置的脊柱植入物。
该装置包括可弯曲为外壳211的一部分或与该外壳刚性连接的骨螺钉210。缆或线218伸出外壳211。缆218被配置在辊216上。辊216与通过旋转齿轮214而能够线性移动的螺杆215相连,齿轮214与螺杆215相啮合。电动机212为齿轮系213提供动力,齿轮系213控制齿轮214的旋转并由此还控制螺杆215和缆218的线性移动。
上述设计允许通过操作电动机212至不同方向而在植入之后的任意给定时间拉动或释放缆218。

Claims (24)
1.一种脊柱植入物,包括:
用于连接至第一脊柱部分的第一脊柱连接件;
用于连接至第二脊柱部分的第二脊柱连接件;和
植入后可变尺寸装置,设置于所述第一和第二脊柱连接件之间,在完成将所述脊柱植入物安装在病人体内的手术之后该植入后可变尺寸装置是可操作的,以导致所述第一和第二脊柱连接件之间的相对移动。
2.根据权利要求1所述的脊柱植入物,其中所述第一和第二脊柱连接件包括椎弓根螺钉。
3.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置改变所述第一和第二脊柱连接件之间的距离。
4.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置改变所述第一和第二脊柱连接件在垂直面和矢状面上的位置。
5.根据权利要求1所述的脊柱植入物,其中所述第一和第二脊柱连接件包括装配有借助于齿轮系能够转动的螺纹杆的第一和第二支撑板,所述齿轮系的旋转改变第一和第二支撑板之间的距离。
6.根据权利要求1所述的脊柱植入物,其中所述第一和第二脊柱连接件包括关于彼此倾斜的第一和第二支撑板。
7.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置是液压操作的。
8.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置是气压操作的。
9.根据权利要求1所述的脊柱植入物,其中所述植入后可边尺寸装置包括与螺杆机构枢轴地连接并通过该螺杆机构移动的铰链臂。
10.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置是电操作的。
11.根据权利要求10所述的脊柱植入物,其中所述植入后可变尺寸装置包括内部植入部分。
12.根据权利要求11所述的脊柱植入物,其中所述内部植入部分包括电活塞,电动机,微处理器,RF发射器/发送器,LVDT,应变传感器,电气线圈,电池和电容器中的至少一个。
13.根据权利要求10所述的脊柱植入物,其中所述植入后可变尺寸装置包括外部控制部分。
14.根据权利要求13所述的脊柱植入物,其中所述外部控制部分包括控制板,处理器,RF发射器/发送器,磁性动力源,电气线圈和蜂窝通信装置中的至少一个。
15.根据权利要求13所述的脊柱植入物,其中所述外部控制装置由密码或口令控制。
16.根据权利要求10所述的脊柱植入物,其中所述植入后可变尺寸装置是可膨胀的。
17.根据权利要求1所述的脊柱植入物,其中所述第一和第二脊柱连接件通过螺杆机构相对于彼此移动。
18.根据权利要求17所述的脊柱植入物,其中所述螺杆机构包括至少两个相同螺纹的螺杆。
19.根据权利要求17所述的脊柱植入物,其中所述螺杆机构包括至少两个不同螺纹的螺杆。
20.根据权利要求17所述的脊柱植入物,其中所述螺杆机构包括多个同心的螺杆。
21.根据权利要求1所述的脊柱植入物,包括处理器和传感器,该处理器和传感器将所述植入后可变尺寸装置作为所述第一和第二脊柱连接件上所施加的负荷或所述第一和第二脊柱连接件之间的距离的函数来控制。
22.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置是可编程的以便根据任意所需负荷和行程限制以任意所需时间间隔激活。
23.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置包括用于以睡眠模式进行操作的电路。
24.根据权利要求1所述的脊柱植入物,其中所述植入后可变尺寸装置由可充电电池提供能量,该电池能够利用远程能量转换器进行充电。
